Fill out and submit the miniDentist questionnaire. If given the option again, click “Find a Dentist.”
miniDentist connects you to a preferred dental professional.
Get examined by the preferred dental office.
Receive your treatment within 30 days, and inform miniDentist of completion via INFORM@miniDentist.com
miniDentist will confirm that the treatment completion and conditions were met.
$100 will be put towards your dental treatment account within 30 days of treatment completion.

Terms & Conditions
This promotion is for a limited time only and for a limited quantity of recipients (100), determined by miniDentist.
This payment is limited to one (1) payment per patient, and for one (1) treatment at a cost of at least $190.
Complete treatment must be performed by miniDentist participating dental offices within 30 days of referral date.
miniDentist (or designated affiliate) will pay the respective dental office $100 towards patient services within 30 days of confirmation of treatment completion. Patient will not receive $100 directly for this promotion.
‘Treatment’ is considered fillings, veneers, crowns, bridges, endodontics, implants, flippers, dentures, cleanings, or extractions.
Exams and radiographs are not services applicable towards the $190+ treatment cost.
miniDentist is not a dental insurance company nor a dental office.
Patient allows miniDentist to confirm treatment cost and completion with the dental office prior to payment.
Upon conclusion of this promotion, this webpage will be retired and applicants will no longer be applicable for offer.
